Pursuant to the California Political Reform Act (Government Code 81000), the following forms are required for web posting within 30 days after receipt of gift or payment.
Form 801: Gift to Agency Report
Discloses payments made to the City when the payment provides a personal benefit to a City official.
Form 802: Tickets Provided by Agency Report
Discloses distribution of tickets or passes to a City official that allows admission to facilities, events, shows or performances for entertainment, amusement, recreational or similar purposes.
